In this sense, it is increasingly necessary to research on the matter from different points of view, both for that purpose, and to alleviate the effects of their symptoms and even delay them. Spain currently stands out in the scientific field of Alzheimer's and Parkinson's by starring in different investigations on these diseases. One of them focuses on transcranial magnetic stimulation (TMS), a technology that until now had only been applied as a therapy against depression in the United States. Transcranial magnetic stimulation allows to establish causal relationships between behavior, cognition and brain activity, and not only correlations, as happens in other neurophysiological or neuroimaging techniques. It also allows modulating brain activity in the long term, which makes it a technique with promising therapeutic applications in those alterations in which it is sought to generate modifications of brain activity.

The director of the Cognitive NeuroLab and the Cognitive Neuroscience and Information Technologies research group of the Open University of Catalonia (UOC), Dr. Elena Muñoz-Marrón, explains to that this is a recent technique. Specifically, it began to be used for the first time in the late 80s and is based on electromagnetic induction. "It generates a magnetic field through a coil that is placed in the patient's brain until an electric current is induced in the brain." Repeatedly stimulating certain brain networks using a magnetic field improves associative memory in humans, according to a study published in the latest issue of "Science." This type of memory allows us to learn and remember the relationship between unrelated elements, such as a specific restaurant and a person with whom we dine there, for example. Although the hippocampus is a crucial area for remembering, it has not been tested directly in humans if it needs to interact with other brain regions for associative memory to be enhanced, the study authors note. Many previous works have focused only on the hippocampus. To fill this gap, Jane Wang and colleagues, from Joel Voss's lab at Northwestern University Feinberg School of Medicine, have tested whether stimulating the hippocampus simultaneously with other brain networks involved in recall increases associative memory. To do this, they have used a technique called transcranial magnetic stimulation (TMS), in which a magnet, by means of magnetic pulses on the scalp, non-invasively activates different areas of the brain and allows to study their function and the interconnections between them.

The results of Voss's group show that TMS increases functional connectivity between the parietal cortex, hippocampus, and prefrontal cortex, among other areas of the brain. And this change in connectivity was accompanied by better execution in associative memory tests. In these tests, the 16 participants in the study, aged 21 to 40, were shown twenty photographs of human faces and asked to remember the word they had read aloud when they first saw each photo. This work is the first to show that, as suspected, remembering requires many brain regions to work in teams with the hippocampus, a key structure for memory. Compared to what happens in an orchestra, magnetic stimulation of these regions would be equivalent to providing the brain with a more talented conductor, which helps a much more precise synchronization of the "musicians". "It's as if we changed the usual conductor for Riccardo Muti," head of the prestigious Chicago Symphony Orchestra, Voss explains. "Brain regions work best together after stimulation with this tectance," he adds.

Therapeutic promise

The researchers believe their work promises to treat memory disorders that take place when the hippocampus is damaged. His approach also has potential for the treatment of mental disorders such as schizophrenia, in which these regions of the brain and hippocampus are out of sync with others, so memory and cognition suffer. Transcranial magnetic stimulation in recent years is attracting the attention of scientists for its promising therapeutic potential. Several studies have evaluated its ability to improve cognitive functions in neurodegenerative disorders, such as Alzheimer's and Parkinson's, and also in psychiatric disorders, such as obsessive-compulsive disorder, among others. In the United States, it has been approved for years to treat some forms of depression resistant to the usual drugs.

However, the work done by Voss's group is different from previous ones: "Other approaches focus on the prefrontal cortex, which is often recruited to compensate for memory loss in various disorders, including Alzheimer's disease. But the cortex itself is not part of the heart of the "memory network," which is the most affected in these disorders. In contrast, our stimulation method targets the core memory network, including the hippocampus," Voss told ABC.

"Our approach will also be different from these studies because we hope to change, in a non-invasive way, the heart of the memory network, including the hippocampus, in patients with memory impairment," says Voss. Until now it was thought that only direct stimulation through neurosurgery was able to influence structures such as the hippocampus, located in deeper areas of the brain, he adds. It is not possible to directly stimulate the hippocampus with EMT because it is too deep in the brain for magnetic fields to reach. So, using MRI, Voss and his colleagues identified a region of the superficial brain, just one centimeter from the surface of the skull, that had high connectivity to the hippocampus. And they checked whether directing magnetic stimulation to this location could in turn stimulate the hippocampus. "I was amazed to see that it worked so specifically," Voss explains.

Better synchronization

With the help of magnetic stimulation, the regions of the brain involved with the hippocampus were better synchronized with each other, as indicated by the resonances performed on the participants. This imaging test records blood flow in the brain as an indirect measure of neuronal activity. And the more coordinated those regions were because of magnetic stimulation, the more able people were to learn new information. The Northwestern study is the first to show that transcranial magnetic stimulation improves memory long after treatment and can be used to enhance recall of events that occurred at least 24 hours after treatment. "Our results open the door to a whole new area of study to see if we can improve memory in people who really need it," Voss explains. This work has been done with healthy participants, but in an upcoming study, Voss wants to study the effect of magnetic stimulation on people with early-stage memory loss. But he cautions that it will take years to determine whether this new approach is safe and effective for patients with Alzheimer's disease or similar memory disorders.
